Form for requests for disclosure of personal data or documents for regions and region-related healthcare providers. In order to reply we need to be able to reach you by calling the region's main switchboard or sending a letter via the region's official address. We release information needed for the region's processing after a request for release has been received and tested for confidentiality.

If the jobseeker has protected identity, do not use the form. Then contact us by phone instead.

Incoming cases are handled in turn order, usually you will receive a response within four working days. The reply is sent by post to the official postal address of the region, and is addressed to the person who made the inquiry.

In order for disclosure of information covered by confidentiality to take place, Arbetsförmedlingen needs a valid registered consent from the customer. If such has not been submitted, you need to start by submitting it and then return with your request for information after three working days.
