What are the certificates used for?
Medical statement certificate (Af 00213)
For jobseekers with ill health, illnesses or disabilities that may call for Employability Rehabilitation Programme measures.
Doctor’s certificate for participants in labour market policy programmes (Af 00251)
For participants in labour market policy programmes who receive benefit in the form of activity support, development allowance or introduction benefit.
What is the purpose of the certificates?
Medical statement certificate (Af 00213)
Qualitative basis for the labour market policy assessment, so that the jobseeker receives the right support.
Doctor’s certificate for participants in labour market policy programmes (Af 00251)
The doctor’s certificate is normally required so that participants in programmes are able to keep their benefit after the seventh day of absence due to illness.
Arbetsförmedlingen can also use doctor’s certificates that have been received as supporting data for adapting participants’ programmes.

The statement is adapted according to Arbetsförmedlingen’s need for information, so that the jobseeker can receive the right offers and service.
The formulation has fewer questions so as to make things easier for the healthcare services.
Both the questions and the medical statement are adapted to conform with the General Data Protection Regulation (GDPR).
Doctor’s certificate for participants in labour market policy programmes (Af 00251)
In accordance with the requirements of chapter 4, sections 1-3 of the Regulation (2017:819) regarding benefits to participants in labour market policy programmes.
The formulation has fewer questions so as to make things easier for the healthcare services.
The doctor’s certificate is adapted to conform with the General Data Protection Regulation (GDPR).
Who must have the certificate?
Medical statement certificate (Af 00213)
Arbetsförmedlingen requests the certificate from a healthcare provider when a jobseeker has consented and stated that they have an established contact with the healthcare provider.
Doctor’s certificate for participants in labour market policy programmes (Af 00251)
The participant contacts the healthcare provider to request a doctor’s certificate; this is a similar situation to when an employee needs a doctor’s certificate to give their employer after seven days’ absence due to illness. The doctor’s certificate is sent to Arbetsförmedlingen.
Who can issue a statement/certificate?
Medical statement certificate (Af 00213)
Arbetsförmedlingen approves the issue of statements by different professions in healthcare. For each individual request, it is the healthcare provider who decides and is responsible for which profession is best suited to issue the statement.
Doctor’s certificate for participants in labour market policy programmes (Af 00251)
According to the requirements of the regulation, the doctor’s certificate must be signed by a doctor or dentist.
Which certificates can the healthcare service receive compensation for?
Medical statement certificate (Af 00213)
Arbetsförmedlingen can compensate the healthcare provider for the actual cost of issuing the statement and for any patient charges.
Doctor’s certificate for participants in labour market policy programmes (Af 00251)
Arbetsförmedlingen does not compensate for the costs of doctor’s certificates. Comparable with doctor’s certificates for employers with sick pay.
